Your health is a priority for Evergreen Valley College and the San Jose Evergreen Community College District. To ensure that we are able to provide support in a timely manner, please complete the
Student Self Reporting COVID-19 Form.
If you have been exposed to a COVID positive person or if you are experiencing COVID symptoms (see list below), please schedule a COVID-19 test as soon as possible. Information about FREE COVID-19 Testing sites in Santa Clara County can be found below.
Upon completion of the
Student Self Reporting COVID-19 Form, you will be contacted by Janice Assadi or a designee from our EVC Health and Wellness Services either by phone or encrypted email (please check your spam). They will follow up on your case and eventually provide the clearance to return to campus. Please note, you cannot return to campus without a clearance letter from our EVC Health and Wellness Services, and you will need to submit a copy of your clearance letter to all your Faculty the same day you receive it.

Please note: When you receive the results of your COVID-19 test, reply to the encrypted e-mail you were sent by Janice Assadi (Janice.email@example.com), and attach a copy of the COVID test that shows your name, the date the specimen was collected, and the resultsOnce she has received the test results, the EVC Health and Wellness Services Office can begin discussing when you can return to campus.

Please note:Once you have been notified by a student, it is crucial to provide the student with the link to the student self-reporting formThis self-reporting form allows Health Services to better prioritize and follow up with students. Once a student completes their self-report, they will receive an email confirmation. Once you receive notification that a student has completed their self-reporting, you may proceed with providing appropriate assistance to the student such as approving absences and providing remote homework assignments. Should you have any questions regarding this protocol, please contact your Dean or supervisor directly.
